Island Health Care, the Vineyard Health Care Access Program and the Vineyard Nursing Association invite you to participate in a community program to help people learn how to manage chronic diseases. This is a free program that brings together doctors, nurses, dieticians, wellness coaches and health educators to provide you with information, support and knowledge on how to better manage these diseases - putting you in control of improving your own health.
The Program is designed to help people who are at risk, diagnosed or in an acute phase of Diabetes, Congestive Heart Failure, Cardiovascular Disease or Chronic Obstructive Pulmonary Disease (COPD).
